Check the plugs - if 2 are dry and one is wet, the wet plug has a bad injector (stuck open).
If two in the bank are wet and one is dry, the dry one is not opening.
Try using a long screwdriver as a "stethescope" and listen to be sure it is opening.
If needed, I have some already cleaned - singles and sets.

I had two injectors go. You've done everything I did, check plugs and wires, injector cleaner, etc. I finally just had to replace them. One was a electronic failure and the other was just totally clogged. Hope this helps you out. FYI, a Haynes manual is a definite help if one is under the intake manifold, and if you are in there I recommend a new plenum gasket since you're in there as well.
