Starting trouble...sorta

ok, on my 99 ford ranger 4.0L 4x4 with 217,000 miles i have had an issue starting for over a year. the truck itself doesn't get driven too much (less than 3000 a year) but lately we have been starting to use it more. so here goes. Truck almost NEVER starts the first time. it cranks and the starter goes and the engine makes the nananananananananananana sound. so i stop, and on the 2nd attempt BAM! fires right up. now if it's cold, the weather i mean, the truck will die. it'll start right and BAM die. if i start it with the truck in neutral it'll idle pretty ok, then put it into drive and BAM it dies. i had a 94 dodge ram b350 15 passenger van act like this and it was a dirty air filter (hella dirty) so i changed filter on truck but to no avail. friend says it sounds like a fuel issue, i happen to agree. because when i go to drive and hit the gas, it's a no go, then BAM! it jumps into gear and starts moving. at first i was worried about my tranny but after warmed up NO problems whatsoever. hence the long wait (a year) before worrying about it. is it a fuel pump? the fuel filter is new. the spark plugs are new. the starter is working just fine seems like. so how come it dies when i put it in gear? but if i hold the gas while shifting to drive it won't die? but it will buck, then after warmed up life is good? i don't understand enough about the truck's fuel system to be able to make a good diagnosis. any help is appreciated!!!
