I've got a JVC HD radio and absolutely loved it when I lived near Seattle. Tons of HD stations there and the clarity is phenomenal! But since I moved to Podunk, WA I don't get any HD stations .
But like Shane said, all you need is the radio and if there are any stations broadcasting in HD you'll get it.

Each receiver is different. Some have a built in tuner, most you have to buy a separate box that plugs in between the antenna and head unit. Either way, it does not require a new antenna. if yours says "HD ready" that means you need to buy the separate tuner from your head unit manufacturer.
I have the Pioneer DEH p5000 (HD ready). I ad to buy the separate tuner for another $100 to make it receive HD radio. I highly recommend it. Almost every station broadcasts in HD now and the sound is CD or satellite quality.

yeah, you still recieve analog. Where I live, almost all stations broadcast in HD, but when you get too far away, the HD doesnt come in and it automatically switches back to analog. Even in analog, an HD receiver sounds better.
As for the HD only stations, the way that works is there are 2 stations on the same frequency. For example, it will show up under 106.7 as "HD1" (the regular station that comes in analog and HD) and "HD2" (the one thats HD only).
