you cant go wrong with Magnaflow! they definitely sound good in real life. its loud enough to draw attention and make the 4.0 sound better than it really is, yet subtle enough to be comfortable on long drives. I had a Flowmaster super 44 for a while, but the droning in the cab was annoying on long drives.

I think the Magnaflo cat-back system is all 2.5" pipe.. not sure though. Check the magnaflow website.
i would go to a muffler shop and get it custom made. its cheaper, plus you can get exactly what you want in terms of pipe size, tailpipe exit, and exhaust tips. I never understood paying almost $500 for a complet kit when it cost me like $150 at the muffler shop to get it custom made

does bigger pipe gain or lose hp?

as far as gaining any power through a cat back exhaust set up, youre not gonna feel any real difference. it sounds good and "might" give you a mile or two better fuel economy, but thats it. pipe diameter is important to a certain degree though to maintain the proper back pressure.

What kind of sound are you wanting exactly? A low rumble, mellow smooth tone, higher toned and almost raspy? I'll post this up to give you an idea of how different the same muffler and truck can sound. The first one is a 3.0 cat back 2.25 40 series flowmaster SI/DO with 3x18" single wall angle cut tips and all cats intact. The second is the same size everything except 4x16" stainless rolled tips and no third cat.

And again, these are CLOSE to the actual sound if you have some good speakers, but you have to be in person to fully appreciate it.
