I average around 18-19 city/22.5-23 MPG highway on my 3.slow. I'm adding an efan as soon as it arrives in the mail. I haven't added underdrive pulleys or a tuner due to cost. I'm thinking some new sparkplugs and wires would help a bit too. Seafoam is the best cleaner for the cost. It might not give you any MPG back, but it will smooth out the ride.
Changing your air intake and exhaust can help with the MPG department as well. If I ever could/would swap out my cap for a tonneau, I'm sure that would help too in terms of MPG as well.

The only mod I have is a DIY ram air setup. I have a 03 xlt, reg cab, manual, 3.73ls, 235-75-15 tires, with 48k miles. Before the ram air I got an avg of 22.5 mpg half city/half highway. After mod I gained 1 mpg. Just seafoamed last month. Gained another 1 mpg for avg of 24.5 mpg. I use a/c seldom and don't horse it at all.

Hi folks. Gas mileage isn't one of my trucks high points either. When bought new in '03 I drove it easy the first few tank fulls, averaged 21.6 MPG (cdn gallons) combined city/highway. Since then I have averaged around 14-15 mpg. I do however admit to being a bit heavy footed while highway driving.
