No power over 3,500

I have a problem that doesnt seem to be getting any better. My truck runs great anywhere below the 3,500 RPM mark. On accelarating, as soon as it goes above 3,500 it loses power and will no longer pull. I can hear and feel the engine stuttering but theres no power on the climb to any higher RPM. Ive tried injector cleaners and ran premuim for a while hoping that it would flush out. No Luck!

HELP PLEASE!

There's no check engine light on. Its a. 98 4x4 5 speed. All stock except a glass pack exhaust.
It doesn't do it in neutral though.

Is this sounding like a fuel or a spark issue? My gas mileage is also suffering.

No I have never replaced the filter. I've had it only a few months but, why would it rev out it neutral and not in any gear?

Yeah that does make sense. I'm hoping that it would be that easy to fix. Any other ideas?
