Here is a link to the instructions from Ford however it shows this to be for a 1999 you need to confirm this information to be true on your 1998 by purchasing a OEM service manual or get a subscription to Alldata or Mitchell on demand diy

Here is a video that shows in detail the instructions which match the Ford information
but it is also for a 1999 4.0
If you have followed these directions and confirmed with a service manual that the instructions work for your year truck and you are still having problems, you may be dealing with a stretched or worn timing chain set.
If your timing chain has too much slack in it you will never get the Camshaft syncronizer installed correctly.
How many miles do you have on the engine?
If you have a lot of miles more than 100k I would suspect a timing chain set may have excessive slack.
Here is a video that shows you how to check for slack in your timing chain.
This video is not 4.0 or Ranger specific yet the information and procedure is the same.
