eBay is going to be the cheapest place to get one probably....i think driveaprerunner (might be slightly different user name) has one for sale that is grey out of his Ranger....there is a thread about it for sale...
as far as how close it is to the headliner at the back of the OHC, Griggs (n3elz) how to says you have to shim it down with pieces of headliner you cut out....luckly i didn't have to....don't know why, but i didn't...

I just installed an OHC. The bracket has a bend on it on the end that points to the rear of the OHC (the end with the screw holes) I only cut a hole in the headliner big enough to barley clear the width of the bracket and slip it above the headliner and just big enough to clear the front clip holes and the rear screw holes. When the OHC is mounted that bend acts to force the headliner down flush with the top edge of the OHC when it is screwed to the bracket sandwiches the headliner between it and the OHC keeping everything nice and flush. I can take a picture later if you need, it might help to explain my ramblings.

If i was you I first try and fix what you already got. Its a whole lot cheaper to buy some new headliner material and a can of spray adhesive and recover the entire thing. Its a little more work but it will save you a good chunk of money. Same thing for you Zadeard. I have done it twice on a F-150 and a XJ. Just make sure to spray the hell out of both sides, the headliner and the new material, before you press them together.
