They're there because 4wd's have a taller front ride height, and there's no frame/suspension differences between the 2wd trucks and 4wds, so a block was used to get the truck to sit right and not *** squatty.
Some guys with 2wd's use stock blocks and just run 4x4 shocks as that literally is a 4x4 setup back there now.
Note: When I say 4x4, I also include 2wd torsion bar trucks.
