Icon4 Rotor issues

Disc rotor where bearings are housed broke and it's not the first time it's happened. I need help figuring out why it keeps happening due to the risk of if happening while driving at high speeds.

Kind of hard to tell from the picture, but it looks like the bearing race is what broke.

Are you running over sized tires ? Wheel Spacers ?

Yeah, I have wheel spacers and 32 x 12.5 BF Goodrich All-Terrain. I currently can't take the wheel off

The wheel spacers and missing dust cap are the biggest reasons you're having this problem, the oversized tires and missing lug nut aren't helping.

Put all 4 things together and you'll keep having this problem.
