I'll tell you what I've been told about this. Somehow this is so you can mount the spare tire of a first gen Ford Ranger in the bed to free up space for a second fuel tank. I can't find one on the internet so I'm not sure how it works but I'm hoping someone out there does. I assume it's rare, then.

There's supposed to be a carriage bolt of some type that goes through the slotted opening. This carriage bolt is long, and will protrude through one of the wheel's lug holes. From there, a large, stamped wing nut threads onto the carriage bolt. It has a rounded bottom so it will "seat" into the lug hole of the wheel.

Usually these also have a support bracket that bolts or rivets to the upper bed rail of the truck so that the weight of the tire isn't tugging the bed's sheet metal apart when the tire bounces.
