Highest I've ever gotten out of my 4.0 OHV was 19mpg. That's with 32's, and about 200 lbs. of tools in my toolbox, lol.

My 2003 Ranger XLT, is 2.3 with automatic transmission. I'm getting around 17.5-18 mpg with city driving and 31 mpg on the freeway.

I don't drive much anymore so the gas prices aren't hurting me too much.

My exhaust did nothing for my mileage...I try to coast when possible, and anticipate stoplights...if it might go yellow I get through it so I don't have to sit for 7 minutes.

If I heard right, someone told me that mythbusters did a show about that showing that caps and tonneaus actually get less gas mileage. I'll see if I can pull some info on the episode...

yup, here is something about it. Can't research more as the school firewall won't let me look at many of the sites.
Mythbuster

This one talks about the tailgate, but I believe they tested toppers and tonneaus as well.
