Trying to find a part name or number

Does anyone know the part name or even the number of what is circled in yellow? The blue represents the leak.

It's part of the heater hose, you buy the whole assembly.

Common problem. If you don't live in a cold climate (and even if you do, it's your choice) you can upgrade to the 2004-2008 hoses which don't have the lines for the heated PCV valve.

Part number and Ebay listings below. When I first replaced these hoses on my truck, I couldn't find both hoses that had the lines for the PCV valve, so I had to go behind the parts store counter and find one that was close enough and make it fit. That only lasted a few years and the plastic fittings started to leak again. So then I replaced them with the hoses below and no longer have to worry about the hoses leaking.
