My truck has a cooling issue?

My truck has a cooling issue?

My 2000 ranger with the 3.0 flex has been having coolant issues. We have 0 clue what it could it be at this point and i need help.
The truck has pushing all the fluid into the burp tank and then blowing it out the tank, after it does that there will be no pressure in the lines but it has also blown both top and bottom hoses. We have replaced every cooling component except for the water but the water is flowing. I have used a pressure tester and it held pressure fine and held a constant pressure when we revved the truck and when we let it sit at idle. We have tried Bars leak and we cannot figure out what is happening. It wont keep the fluid in the radiator, it will push all the coolant to the burp tank and then blow it out the burp tank when its full. We have flushed the system twice now. I have no clue what it could be

First question is have you replaced the radiator cap? I have an issue when using the Torque program to monitor temps, but I'll start my own thread.When I go my current 98 3.0 truck, i changed all the fluids, flushed the coolant, and put a new replacement cap on the radiator, Factory gauge never has passed right at the middle, and never had it actually over heat, it has pushed some funky stuff into the overflow tank, but that seems much better now that the flush was done and the heater control valve replaced.

I have replaced the cap, the cap is working properly. It holds pressure fine and I've never had it over heat before until Tuesday when it blew through the bottom hose. I have no clue what it is. I've flushed the system. I've replaced sensors, thermometer, and hoses. It pushes all the fluid into the burp tank and then it'll spit all the fluid out the tank.

Reads like a blown head gasket or cracked head
