Loosing power going up hills or slopes

Hi, I have a 2003 ford ranger 3.0 V6 and recently when I go up hills it Boggs down and if I hit 3K rpm the CEL starts flashing on it. It also makes a noise like something is coming loose on the bottom or hitting when you try to force it up the hills. When I read the codes it says misfire cyl 4-5. But on flat ground the CEL goes away I have tried almost everything I would check and don’t know what the issue could be. I have replaced the spark plugs and cables. Changed out the fuel filter. Someone told me that it could be the cam synchro another one said it could be the evap system. So if anyone has any suggestions or has deal with this. Any advice would be appreciated. She has 150K miles.
