belt definitely came off. i had a spare so i swapped them out, and when i started it up, there was a loud squeaking. shut it off, looked, belt is sliding off again. noticed my idler pulley is a little crooked. but now, my tension pulley isnt providing any slack. any ideas on this?

Replace both. Sounds like one or both failed. Pretty common, especially with high mileage, but not unheard of with low mileage either.

+1 ^^^

Also spin other pulleys to make sure they turn, alternator, water pump and A/C compressor, power steering pulley should move but very hard to turn this pump sometimes
