Rough Low Idling!!!

i read up on all the posts and none of them mathch my problem. My motor only does it when i get off the interstate or drive a long time. What could cause this?

i had this happen to my truck did you check the icm. im not a 4.0 im a 3 but could still be the same problem.

wat is the icm?
idk much about vehicles. im young lol maybe sum pics would help?
