Clutch Disengagement

My father in law has a '93 Ranger with the 4 banger, 5 speed. He said that when he goes to shift gears, it feels like the clutch is not disengaging, and it is hard to get into the next gear. I know that the clutch system on these trucks is self adjusting. Sound like a major problem, new clutch/hydraulic system?
