Clutch? Tranny? or Fluid?

Today i was on the highway going almost 70mph. I was in 5th gear cruising along and BAM, i heard a very light grinding, still in 5th gear, foot on the gas and then engine went up in rpm's. I threw in neutral as fast as i could, downshifted to 4th. everything seemed ok after that. this was the 1st time this has ever happened. got to where i needed to go. a few hours later same picture... on the highway 5th gear issue then downshifted into 4th and it did the same as it did to 5th gear! I had to actually pull over to make sure all gears weren't affected. 1-3 gears were ok under normal driving on the streets, then 4th was ok then 5th was ok. this only seemed to happen on the highway at high speeds. i also stopped in a parking lot to "test" the clutch, no slippage, but got the usual burning smell when testing. so im wondering... am i in the beginning stages of needing a new clutch or tranny? maybe low fluid? if it was low on fluid, how do i fill it up? All i see is the drainage screw and the transfer case screw. would these be the placed to fill it up or do i need to take the truck in to get it filled? i figure id get some basic steps to do first before the ending result of needing a new tranny or clutch. thx in advance.

---forgot to add that its a 93 manual shift.

look on the drivers side of the transmision there will be a large size hex head with the words "fill" next to it. pick up a haynes manual for your truck. best $30 you'll spend.
