grind into third

Driving down the road after the truck warms up and has been drivin for about 10 minutes, coming from second or downshifting from fourth, it grinds. If you put it in neutral for a few seconds, it goes in, but acts like its going to throw back out. My uncle is coming to take a look at it Saturday, and from describing it that way, he says the synchro is going out. Anyway to replace that, any other ideas as to what it could be? Trucks a 99 3.0 manual with 138k on the clock.

try to change the fluid to a good synthetic ATF like mobil 1 or amsoil... mine had the same problem and that did the trick
