Manual Transmission Problem
I happened to get mine on a deal weekend, but the normal price on one around here I think is $150-200... That's if you pull it yourself, and if you can use a wrench, you can pull it yourself... I know Shane and I have both dropped tranny's alone in the past.. It's not fun, but can be done.
I don't see the reverse synchronizer causing this problem.
Have you checked the fluid level in the transmission?? You might have lost fluid while it was on its side. If so, then the trans is probably lunched. There should have been a lot of red fluid around if that happened.
I would also check for a broken transmission mount, loosened u-bolts on the rear axle or broken bolt in the center of the spring packs.

That clarification changes things. I'd say that you have bigtime internal transmission problems. You could pull the top cover off the tranny to see if your fall to the other side of the cab damaged the shift mechanism somehow but that's a long shot. Other than that, I think the transmission has to come out for a teardown or swap. If you go the swap route, a manual replacement trans needs to come from a 2001+ 4.0L Ranger. That's because of the bell bolt pattern and the M5ODR1-HD internals that were upgraded for the 4.0L SOHC.
The 4x4 extension housing is easy to change but I'd check into the output shaft before buying. Those are usually different 4WD to 2WD and swapping it probably requires a transmission teardown.
