DO a search on vaccum leaks for your engine,Find the leak,Replace the line, reset the codes.

There should be a sticker either on the underside of the hood or on the rail in front of the radiator with a vac hose diagram . Just follow all the hoses from the engine and look them over carefully.
The most probable place to look is the hoses from the charcoal canister to the engine.
As the engine pulls vapors from the fuel tank , the gas vapors tend to deteriorate these hoses faster than others.

It most likely won't be a 'hole'.
Check where the hoses connect for cracks / splits. And also near the exaust manifolds where one might be melted through.
