Welcome to the forum

These are mostly unrelated except they are light related

1991 has probably had one or two DIY trailer light wiring jobs, I would start there

Running lights are coming on because brake lights have no grounds, so those bulbs are using the running lights ground
Brake light grounds are in the rear with trailer light wiring splices and park light grounds
Might not be the problem but anytime there is an exterior lighting issue on an older vehicle, always start at the trailer light hook ups, they are exposed to the elements and deteriorate over time

1991 has two flasher units, one for turn and one for 4-way
On the front of the Cab fuse box is the round Turn signal flasher
On the BACK of the fuse box is the 4-way flasher, same model as the front, interchangeable

You can just buy one and swap out the one on the back, or swap the two you have around and see if turn signals stop working then replace that flasher
May not be why the 4-ways are not working but its the most common reason
