Concerning noise

I recently purchased a 1995 xlt super cab ranger with a rebuilt engine and new clutch . I assume the guy kept the same tranny. I noticed when it’s in 1st , 2nd and 3rd gear it makes a noise I think is coming from my clutch . When I accelerate it sounds like something lose is spinning. When shifting from 3rd to 2nd The noise increases . 4th and 5th gear dont have the issue .

You have an M5OD-R1 transmission, aka M5R1, thats the model number not what engine it fits, just FYI.

They are pretty bullet proof.
Clutch noise should change when pedal is down all the way or up all the way
Which you don't mention?

If noise changes with pedal position then yes, could be throwout bearing or pilot bearing in the clutch system


4th gear is not really a "gear", its 1:1 ratio, meaning engine RPMs(input shaft) and driveshaft RPMs(output shaft) have same RPMs, so when you shift to 4th "gear" you are just moving a sleeve that connects the two shafts together no "gear" used.
This is important because the Input Gear in the transmission is not used in 4th so no stress on it like there is in 1st to 3rd, and 5th
But 5th is an overdrive so not the same stress there is in 1st to 3rd.

One problem M5R1 trans had was input gear and bearing issues, not common at all, but if M5R1 had a problem that was usually the problem it had

Not simple to diagnose that outside of pulling out the transmission and having a look inside

I assume you have checked fluid level in transmission, it used ATF, yes automatic transmission fluid(Mercon V) in a manual trans.
