Serpentine belt keeps coming off!! Tensioner most probably??

Thats what mine did when it was going out so I'd put my money on that. Sometimes you can tell where the point where it is falling off at by looking at the belt system while running from the side and you can see what pulley the belt wants to come off at because over time the tensioner will wear out and not be true. Thats just my experience with my 3.0 belt system hope it helps.
