put a all-thread connector on the top of the lever where the **** goes on, insert a 38"-24 bolt that has the end cut off, use a jam nut on both ends and put the **** back on. you can leave plain or slide a peice of hose over the extention. i dont have a lift but thought the lever needed to be longer. this was quick, cheap and easy.

You can find more info here:

You dont even need to get technical with it. A friend of mine and Trevor's has his going straight up using thin pieces of metal on each side of the hurst 'stub', give it approx 3" straight up extention(alot like my tcase extention below). IMO...thats still too much of a reach for me.
AAAAAAAND here's mine... 2.5" up, 1 1/8" back, 1/4" towards the driver(because of how it's mounted to the hurst). Trepaul's has the same measurements.
I LOL'd at that pic Marcus posted....I sent that via cell while I was bored at work! HAHA!

OH!
And since you're bodylifted...DO NOT cut your floor rubber piece as pictured in Trevor's pic.....you dont need to. With the extention, The seal is actually VERY nice and it's quieter in the cab.
Trevor...I'll drop off that one you got me so you can quiet the inside down of yours since I obviously dont need it anymore. lol.
