After you turn the bolt all the way out until it seems to just spin, put the jaws of a pair of channel locks under the head of the bolt and give the jaws a couple of taps with a light hammer. This will seat the final two threads and you will be able to turn the bolt all of the way out. If it still spins use the channel lock jaws to exert some outward pressure on the bolt head while you try to turn it out by hand. If you have to remove the access panel use a small drift or punch to tap the base of the bolt. It should not take more than a couple of light taps. The bolt is not captive and there is no clip or keeper inside the tailgate. The nut is captive and it will not fall off inside unless it had been damaged by someone who is not mechanically inclined.
