Ford ranger 94 big problem

So I’m having a problem with my 94 Ford ranger v6 4L everytime I am pushing my gas all the way to the floor it hesitates over and over sitting at 25 mph not going up at all when I hold my foot on there it’s like I’m pushing on the gas and pulling off over and over again and about 7 seconds in of holding your foot down it makes a loud noise like something is dropping I have no clue what this is in my truck or what to do anything can help

Manual or automatic transmission?

Do you have a tachometer on the dash?

When stopped, and in Neutral or Park, can you REV engine OK?

Is the check engine light on, and if it is, have you checked for codes ?
What modifications have you done to it, and did you notice the problem after one of the modifications ?
