Rough idle with click then tick and squeak?

My ford ranger is experiencing a pretty bad idle mainly in drive. Seems to be tied to a few noises that kind of come in packs. It will start with an audible click and a squeak. It will then tick loudly for a few seconds then run smooth and repeat. Spark plugs changed made a difference for less than 4 hours then it reverted, when I changed spark plugs they looked fine. I only really have a video to offer as there is no lights or codes. My guess is cam synchronizer but idk I’m new to rangers any idea of what the noises could be if anyone has any ideas I would much appreciate it.

It also seems the poor idle is much more noticeable with ac on, turning it on and off makes an immediate difference, like the second I flip it.

Turn the fan , ac and temp control to off. See if the clicking stops.
