Hello all!
I have had this issue with my engine for a while now that has been really bothering me. Whenever it is at a complete idle, i.e. at a stop light, the engine runs a little rough. It's nothing that'll remove fillings, but I notice it. Instead of the idle being a smooth pattern, like a sound wave, it will sound (and feel) like it fluctuates, albeit with no change on the tachometer. When my mechanic replaced my cracking front fan, the idle changed a bit. There are groups of vibration like a normal idle, but it goes out of sync after 5 rounds or so. I just recently tried changing my spark plugs and that changed nothing. I have also had the truck checked for vacuum leaks multiple times by multiple shops. Any advice/ ideas are welcome!

And the mechanic change the Fan Clutch along with the fan blades?
When engine is STONE COLD
Remove fan belt from crank pulley
Start engine, you can only run it for 2 min with no water pump, Battery light will be on, thats OK
See if the vibration is gone
Engine bay will be VERY quiet with no fan, so listen for misfires
Shut engine off, as soon as possible
If vibration was gone and no misfires heard then spin the accessory pulleys to see if any feel "odd"

What does a misfire sound like? I am new to that.
Rough running, not smooth, this would be a spark or fuel issue, but tach RPMs should change when there are misfires, and if 1995 or up Ranger then computer will set minsfire codes and turn on the CEL(check engine light) if it happens often enough
Rough idle can often be cured with a can of Seafoam, or similar injector cleaner, added to the gas tank once a year, dirty injector tips cause fuel to drip out instead of spray out at idle RPMs

mine does this too. I replaced damn near everything trying to make it go away, its exactly as you described in the OP

fuel system cleaner
fuel filter
clean TB
clean MAF
PCV valve
EGR
sprayed for vacuum leaks
IACV replaced
ECT
flush the cooling system and thermostat replaced
and probably some other bullshit I missed
and it still does it lol. GOOD LUCK OP, tell us if the fan trick worked because maybe thats what I need to do.
